About Family Foundations

Family Foundations is a family support program offered by Pocahontas County Home Care Aide Agency to assist pregnant women and families of newborns and children aged two at the time of admit. Family Foundations will focus on prenatal care, child development, child health, parenting skills and other aspects of family functioning.  The home visitation program will help your child with being school-ready and educational and fun activities are planned weekly until your child begins kindergarten.

The purpose of Family Foundations is to help families prepare for a new baby and to provide the best care possible after the baby arrives.  Your Family Support Worker will be a person you can count on.  The worker will provide support as you make choices for you and your family.

Your Family Support Worker can help you give your baby the most valuable gift of all - a loving, nurturing parent. The worker will teach you ways to care for your baby that will help him/her become the best he/she can be. Your Family Support Worker will visit you in your home or at another place of your choice.

Family Foundations is provided at NO CHARGE to participating families residing in Pocahontas County! It is funded through a grant from Linking Families and Communities through the state Empowerment program.

Contact the Pocahontas County Home Care Aide Agency at 712-335-4587.  A Family Support Worker will provide a home visit and assist in the completion of an enrollment application.
